Our story

Trelity Health began in December 2021 as a small two-person team providing assistive services to people living with disability. Within six months, the service grew quickly, and the business moved into a dedicated office in Croydon Park, SA, giving the team a stronger physical presence and more room to support clients in meaningful ways.

In May 2023, the first staff member was fully employed, and since then we have welcomed new clinicians across Occupational Therapy, Physiotherapy, Exercise Physiology and Support Coordination. Today, we have grown into a committed six-person team.

This growth reflects our clear purpose: specialising in progressive neurological conditions while supporting all adults living with disability. Trelity Health is now a multidisciplinary allied health and clinical care provider offering Occupational Therapy, Physiotherapy, Exercise Physiology, Nursing, Support Coordination, professional education, specialised equipment hire, and information and support with accessing the NDIS, including pre-planning.

Founders and clinical leadership

Carlye Smith and Jerry Packer established Trelity Health to help people in South Australia living with progressive neurological diseases or disabilities achieve greater independence and quality of life.

Carlye is an experienced Occupational Therapist who supports people with conditions such as Motor Neurone Disease (MND), Multiple Sclerosis (MS), Parkinson’s Disease, Dementia and stroke recovery. Jerry is a Registered Nurse with deep knowledge of neurological conditions and the NDIS.

Together, they have created an allied health service focused on genuine care, effective communication and connected supports that make daily life easier. With an in-depth understanding of neurological conditions and NDIS processes, Carlye and Jerry guide clients from initial application through to ongoing care, preparing the necessary evidence, coordinating allied health and community supports, and maintaining consistent communication across providers.

What our name represents

The name Trelity Health was chosen to feel inspiring and memorable. It naturally conveys care and moving forward, the exact message we want people to feel when they hear about us or work with us. There is no other Trelity Health.

Our team

Trelity Health was founded by Carlye Smith and Jerry Packer, who share a passion for empowering people to live independently and with confidence. They believe genuine care and clinical expertise can transform how support is delivered.

Today, the Trelity Health team includes registered occupational therapists, physiotherapists, exercise physiologists, nurses, and support coordinators, each committed to person-centred care and strong communication.

Carlye Smith

Occupational Therapist

Meet The Team

Carlye Smith – Occupational Therapist

Carlye Smith is an experienced and sought-after Occupational Therapist based in Adelaide.  With almost two decades in Occupational Therapy, Carlye specialises in helping patients with MND, MS, Parkinson’s and those recovering from a stroke. Her extensive experience in progressive neurological conditions means that she can anticipate clients’ needs to ensure all the right supports are put in place before they become critical.

Assessment & Reporting

Carlye spends most of her time out in the community assessing client’s function and making recommendations to ensure that clients gain the support and equipment they need quickly and efficiently.  Her clinical expertise and functional capacity assessments have been instrumental in helping clients gain access to the NDIS and securing additional funding when their needs change.

From providing prescriptions for wheelchairs, lifters and hoists to specifying and organising complex home modifications (simple and complex), Carlye is passionate about ensuring you get the best help and support possible.

Outside of work, Carlye loves to spend time with her family and friends.  Her son is heavily into sports so she can regularly be found helping out at the local football club and scoring cricket matches.  Carlye enjoys reading and walking and loves diamond art.

Jerry Packer

Support Coordinator

Meet The Team

Jerry Packer – Support Coordinator

Jerry is a qualified registered nurse and has been working as a Support Coordinator since the rollout of the NDIS in South Australia. His clinical background and in-depth knowledge of MND and neurological conditions enable him to provide both general and specialist support coordination.  His years of experience in the field of progressive neurological conditions means he has a huge network of skilled and trusted providers to deliver the specialist care and attention that participants need.

Experienced In Motor Neurone Disease (MND)

Jerry has extensive experience with MND and has been a committee member of an MND research group: TRG.  He spent a short time as the interim CEO of MND SA and was on a national review committee that liaised directly with the NDIS to negotiate and discuss service provisions for participants with MND.  He has developed and delivered a range of workshops on MND as well as assisted MND Australia to design and review a range of different MND resource materials.

Extensive NDIS Knowledge

Jerry has extensive knowledge of the inner workings of the NDIS having set up NDIS services and processes in various organisations.  His in-depth experience and knowledge of how the NDIS system works has been vital in helping to secure additional funding for clients.

Jerry is a family man with two children.  He loves the outdoors and camping and is a regular volunteer with the local Surf Life Club.

Nick Taylor

Support Coordinator / Exercise Physiologist

Meet The Team

Nick Taylor –Support Coordinator / Exercise Physiologist

Nick is an Accredited Exercise Physiologist and has recently made the transition into a Support Coordinator role after developing a passion for working within the NDIS space throughout his role as an AEP. Nick is also very computer literate, having picked up many skills throughout his time studying & doing lots of his own administration work while working as an AEP. Known for his easy-going, laid-back nature, Nick is always happy to help with anything he can. He also loves a good chinwag, and most importantly enjoys making people laugh, and feel comfortable around him.

Allied Health Experience

Nick been an Accredited Exercise Physiologist since 2020 and has almost 4 years clinical experience in this space, working with a wide variety of clients including DVA, LSA/Work Cover, NDIS & Support at Home (Aged Care). Nick completed a Bachelor’s degree in Sport & Exercise Science, before going on to complete a Master’s Degree in Clinical Exercise Physiology. Nick can lean on his Allied Health knowledge & experience to ensure his participants are receiving the best possible care, and also understands the challenges that a participant with progressive neurological disease may face, given he has seen these firsthand throughout his time working with the NDIS sector as an AEP.

Outside of work, Nick is a sport’s nut. During the cooler months, he plays soccer and watches as much AFL & NRL as he can. During the summer, he plays Cricket but will spend time watching, listening to, or reading about most sports imaginable. In his downtime, you will usually find Nick playing his PlayStation, which involves mostly sports based games (shock!).

Luc Scheepers

Meet The Team

Luc Scheepers – Occupational Therapist

Luc is a passionate Occupational Therapist committed to delivering high-quality, person-centred care, with a strong focus on community participation and social engagement. With experience across a range of practice areas, including paediatrics and mental health, Luc brings a broad and flexible perspective to his work, allowing him to tailor support to everyone’s unique goals and circumstances.

Luc values independence, self-identity, and personal freedom, and these principles guide his approach to assessment and intervention. He supports clients with assistive technology prescriptions such as wheelchairs, ramps, and slings as well as home modifications and functional capacity assessments to support updated NDIS funding.

Outside of work, Luc enjoys staying active through sports including rugby union, cricket, and pool. When he’s not on the field, he unwinds by playing video games and spending quality time with family and friends.

Aballa Opio

Physiotherapist

Aballa Opio – Physiotherapist

Aballa is a dedicated physiotherapist, enthusiastic about keeping the community independent, active and confident for as long as possible.

Aballa has strong foundational clinical experience, skilled in land and hydrotherapy based therapeutic exercises, evidence-based patient education and a strong focus on collaboration and patient advocacy. She has comprehensive experience supporting physiotherapists in patient care, through her previous role as a Level 2 Physiotherapy Assistant within SA health.

Aballa is particularly passionate about rehabilitation and exercise-based therapy that supports meaningful, functional goals. She is an empathetic character who recognises participants vulnerable circumstances whilst remaining committed to empowering participants to maintain dignity, independence and quality of life. She takes pride in helping clients exceed their expectations, rediscover their physical capacities and building trusting relationships with clients and those involved in their care.

Aballa is a friendly and sociable character, who outside of work, enjoys dining at the best restaurants in Adelaide- always up for a chat about where to go for her next meal.

As a physiotherapist, I am committed to delivering client focused and goal-orientated care tailored to clients needs and functional goals.

I provide:

  • Comprehensive initial assessments to inform personalised evidence-based exercise programs
  • Objective outcome measures to measure and track progress
  • Clear communication and care continuity by detailing and communicating progress reports for the NDIS, support coordinators doctors and other allied health.
  • Flexible service delivery offering both home-based and in-clinic appointments
  • Hydrotherapy and land-based services
  • Manual therapy services where clinically indicated
  • Manual handling and transfer assessments with education for family members and support workers to ensure safe transfers specific to the participant
  • Development of tailored home exercise programs with education for participants and support workers to facilitate exercises
  • Mobility and advise gait aid prescription
  • Gait retraining for participants requiring weightbearing support via use of hois

Isaac Smith

Support Coordinator / Administration Assistant

Meet The Team

Isaac Smith – Support Coordinator / Administration Assistant

Isaac is our support coordinator / administration assistant. He provides organisation, managing office operations, and providing support to the team.

He assists with administrative tasks, document preparation, and proactive completion of tasks, making him a trusted resource for colleagues and supervisors. He is also in charge of the social media which will allow Trelity Health to grow.

In his spare time, Isaac is very sports orientated and enjoys hanging out with his friends and family.

Looking ahead

Looking ahead, Trelity Health remains focused on providing person-centred care for people living with neurological conditions and disabilities, as well as those managing complex or chronic health needs.

We recognise that funding pathways can be confusing, whether support is accessed through GP Plans, the NDIS, Support at Home (Aged Care), or private funding. We aim to make these pathways more transparent and more manageable, so people can focus less on paperwork and more on living well.

Trelity Health will continue to develop and refine services that bring together Allied Health and Nursing, expand clinical programs, and build strong partnerships with community organisations.
