What Is Support Coordination?

Support Coordination is an NDIS funded service that is designed to support participants to make the most of their NDIS funds.  Support Coordinators help and empower you to understand your plan and build your confidence so you can spend your funds in the most appropriate way. 

No two people are the same and your NDIS funding is there for you to spend on accessing the right care and supports to help make day to day living easier, safer and more manageable.

Listening To Your Needs & Giving You Choice & Control

Once we’ve looked through your plan, our Support Coordinators will help you work out the best and most beneficial way to use your funds.   We’ll look at the overall budget and talk you through what can be achieved and recommend the best options for you.  You can then choose which services you want to use and have full control over how your budget is spent.

Core Supports

The types of Core Supports you can access depend on your circumstances. Core Supports cover a broad range of services including care and domestic support like cleaning, ironing, meal preparation and gardening as well as transport and activities to get you out and about in the community. Core Supports funding can also help with consumables and low-cost equipment and our team will advise you on exactly what this covers.

Capacity Building Supports

are all about helping you build the skills and capacity to achieve your goals.  Support Coordination is included as part of Capacity Building along with community nursing, allied health therapy, counselling, training and employment services.

Capital Supports

Not all NDIS participants can access Capital Supports but many participants with progressive neurological diseases can.  Capital Supports include wheelchairs and mobility aids, assistive technology and complex home modifications such as bathroom remodelling.

Once you understand your plan and your entitlements, your Support Coordinator will help you choose the best service providers to suit your needs.  We’ll work with you to ensure you have a good mix of supports to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Connecting You With The Providers You Want to Work With

Once you’ve decided who you want to work with, we’ll set up meetings with potential service providers.  This allows you to meet the people who’ll provide your care, get a feel for the services they offer and see if they are the right fit for you. 

Implementing Your Plan & Service Agreements

We like to make your NDIS journey stress free and easy so once you have met the providers and decided who you want to work with, we’ll take care of all the paperwork.  We’ll negotiate the costs and set up the relevant service agreements and booking arrangements, so you don’t need to worry about a thing.

Trelity Health is excellent at planning budgets and will design your supports to last the life of the plan, so you’ll always have continuity of care. 

Regular Feedback & Monitoring

Our friendly Support Coordinators always go the extra mile to ensure your needs are being met and you are getting the support you require.  We’ll regularly check in with you to make sure everything is running to plan, and you are happy with the providers you have.  If your needs or circumstances change, simply let us know and we’ll work with you to change your plan.

Help With Your Plan Review

NDIS plans often run for between 12 months to 3 years.  Before your plan comes to an end, we’ll review your existing plan and work with you to define what you need in the next round of funding. 

The team at Trelity Health will undertake all the pre-planning work and can attend your planning meeting with you.  This way we can present a full overview of the different supports you require, the level of funding needed and all the documentation (provider reports and capacity assessments) to support your case to help you secure the level of funding you need.  

Understanding the levels of Support Coordination

In the NDIS, Support Coordination is organised into different levels. Knowing which level aligns with your plan helps you get the proper support and funding.

Level 2: Coordination of supports

This is the most common level for participants who need practical help understanding their plan, connecting with service providers, and implementing supports.

Level 3: Specialist Support Coordination

Designed for people with complex needs, high risk, or multiple providers. It ensures advanced coordination and service integration across environments.
