We support NDIS participants with a wide range of disabilities, from all walks of life and want to help and empower you to live your best life. Our team specialise in progressive neurological diseases and work with participants and families affected by Motor Neurone Disease (MND), Multiple Sclerosis (MS) and Parkinson’s.
Helping Self-Managed, Plan Managed & NDIA Managed Participants
Trelity Health can help match you up with the right mix of supports to assist you to build capacity, retain your independence and achieve your goals. We work with clients who are self-managed, plan managed and are currently applying for registration to help NDIA managed clients. We’ll listen to your needs and help match you up with the right supports and providers to optimise your care and get the most out of your funding.
