Trelity Health provides home-based Nursing care delivered by an experienced Registered Nurse with deep experience in neurological and complex health needs. Nursing support focuses on safe, practical clinical tasks such as wound care, continence and catheter management, diabetes management, PEG and enteral feeding, respiratory support, bowel care, and medication oversight.
Services are provided in the person’s home or in residential aged care settings, with telehealth available when appropriate. Nursing works closely with our Occupational Therapy, Physiotherapy, Exercise Physiology, and Support Coordination teams to keep care connected from assessment through to ongoing review.

Support can be provided for residents in aged care homes for clinical needs arranged privately or through existing funding. (Services do not replace facility-based Nursing staff.)
Telehealth Nursing
Used for appropriate follow-up, education, monitoring, and review when hands-on care is not required.
Community environments
Clinical education, training, or consultation can occur in community locations where suitable and safe.
Nursing availability and NDIS requirements
Our Nursing services are delivered at home or in independent living environments. We do not normally provide Nursing inside residential aged-care facilities, as these settings normally have their own clinical teams.
For NDIS participants, Nursing can be arranged only when the participant’s plan includes the appropriate health-related supports. Our team can advise on eligibility, documentation, and how Nursing fits within broader therapy and daily support needs.

Do you provide Nursing services in aged care homes?
Yes. We provide privately arranged nursing services for residents in aged care settings. This complements, but does not replace, the facility’s internal nursing staff.
Do you offer Nursing through the NDIS?
